Muscle groups inside a distorted skeleton, with organs on the outside. The trickiest or most confusing things would be the boney sockets on the skull would bulge out (shielding the eye balls which would be completely encased by them)… Also the septum of the nose would be on the outside of the nose area of the skull… I guess the brain would be like a hat?
Nah you over think it. All that needs to be accomplished is for what’s inside to become outside.
A laxative would count under this very literal interpretation.
